Liquid Personnel is looking for a dedicated Social Worker to join our client's Home, Independence and Crisis Support (HICS) team, helping people return home safely and confidently through a Home First / Discharge to Assess approach. This is a fantastic opportunity to work at the heart of an established partnership across health and social care in Dorset.

What will your responsibilities be?

You’ll play a key role in supporting timely, safe hospital discharges and delivering high‑quality social work practice. Your responsibilities will include:

  • Visiting acute and community hospitals across Dorset to facilitate effective discharge planning.
  • Completing strengths‑based Care Act assessments and complex discharge assessments.
  • Managing risk and ensuring safe transitions back into the community.
  • Developing personalised care and support plans, coordinating services such as reablement, intermediate care, equipment, and long‑term support.
  • Working closely with NHS teams, therapists, GPs, housing partners, and the voluntary sector to ensure seamless care.
  • Addressing safeguarding concerns in line with Dorset Council procedures.
  • Applying the Mental Capacity Act, completing capacity assessments, and leading Best Interests decisions.
  • Managing a busy workload and supporting weekly duty in the office.

Qualifications & Experience:

  • Qualified and registered Social Worker (degree or diploma).
  • Experience in hospital discharge or complex casework is highly desirable.
  • Strong understanding of relevant legislation, safeguarding, and strengths‑based practice.
  • Comfortable working with complexity, including Court of Protection matters.
  • Enhanced DBS (Children’s & Adults) – obtained by us.
  • Right to work in the UK.
