Liquid Personnel is recruiting Social Worker for its client's Adult Community Team at Wiltshire.

This is an exciting opportunity to work within a compassionate and person-centred service that supports adults aged 18 and over who are living with physical disabilities, chronic and long-term conditions, autism, and sensory impairments.

The successful candidate will join a supportive and collaborative team where wellbeing, respect, integrity, and professional development are highly valued. You will work closely with individuals, families, carers, health professionals, and community partners to deliver strengths-based interventions that promote independence, dignity, and positive outcomes. This role offers the opportunity to manage complex casework whilst contributing to best practice, safeguarding adults, and supporting the ongoing development of colleagues within Adult Social Care.

What will your responsibilities be?

  • Undertaking complex, person-centred assessments to identify eligible needs, strengths, risks, and desired outcomes.
  • Managing a varied caseload of adults with physical disabilities, sensory impairments, autism, and long-term health conditions.
  • Completing Care Act 2014 assessments, reviews, and support planning to promote independence and wellbeing.
  • Developing, implementing, and reviewing person-centred Care and Support Plans that reflect individual needs and outcomes.
  • Undertaking Mental Capacity Assessments and leading Best Interest decision-making processes where required.
  • Progressing safeguarding enquiries, including Section 42 enquiries, and ensuring vulnerable adults are protected from abuse, neglect, and exploitation.
  • Assessing and managing risk through strengths-based and proportionate interventions that support positive risk-taking and independence.
  • Supporting carers in complex situations and ensuring carers' needs are appropriately identified and addressed.
  • Working closely with multidisciplinary teams, NHS colleagues, housing providers, voluntary organisations, and other partner agencies to coordinate effective support.
  • Preparing high-quality assessments, reports, court documentation, and professional records, including Court of Protection reports where required.
  • Representing Adult Social Care within multidisciplinary meetings, safeguarding meetings, and other professional forums.
  • Providing advice, guidance, mentoring, and informal supervision to colleagues, newly qualified social workers, and students where appropriate.
  • Maintaining accurate, timely, and legally compliant case records and documentation.

Qualifications & Experience:

  • Professional Qualification: MA, BA, BSc in Social Work, Diploma in Social Work, or CQSW.
  • Registration: Must be registered with Social Work England.
  • Experience: Significant post-qualified experience within Adult Social Care, including complex case management and safeguarding work.
  • Legislation Knowledge: Strong understanding of the Care Act 2014, Mental Capacity Act 2005, Continuing Healthcare guidance, Court of Protection processes, and Adult Safeguarding legislation.
  • Assessment Skills: Proven experience undertaking complex assessments, reviews, support planning, and risk management.
  • Safeguarding Experience: Experience undertaking Section 42 enquiries and managing safeguarding concerns involving vulnerable adults.
  • Multi-Agency Working: Strong experience of working collaboratively with health services, community organisations, carers, families, and partner agencies.
  • Communication Skills: Excellent interpersonal, negotiation, report-writing, presentation, and decision-making skills.
  • Professional Development: Commitment to continuous professional development and supporting the learning and development of others.
  • Driving Licence: Full UK driving licence and access to a vehicle is desirable due to the community-based nature of the role.
