Description
A Local Authority mental health social work team is seeking an experienced Social Worker to work alongside health colleagues within a Community Mental Health Team based at Dewi Sant Health Park, Rhondda Cynon Taf.
What will your responsibilities be?
- Assess and support citizens within a mental health setting
- Work collaboratively with multidisciplinary health teams in a CMHT
- Complete Social Services and Well-being (Wales) Act assessments and support planning
- Carry out Best Interest (BI) decisions and Mental Capacity Act (MCA) work
- Lead on safeguarding enquiries and interventions
- Provide duty cover as required
- Attend Quality Assurance panels for funding provisions
- Support effective hospital discharge processes and community transitions
Qualifications and Experience:
To be successful in this role you must have;
- Proven experience within mental health or CMHT settings
- Strong working knowledge of the Social Services and Well-being (Wales) Act, MCA, and safeguarding frameworks
- Ability to manage a complex caseload in a fast-paced environment
- Excellent assessment, communication and decision-making skills
- Available to start immediately
